Opt out of military contact reporting program

Each high school receiving federal funding is required to provide data about their 11th and 12 grade students to military recruiters. Under the law ( Federal public law 107-110, Section 9528 of the Elementary and Secondary Education Act of 1965, as amended by the No Child Left Behind Act of 2001) students or their parents/guardians have a right to refuse participation in this process by completing an Opt-Out request. The personal information of students who opt-out is omitted from the data we provide to military recruiters.

This form is to allow students to opt-out of this program so that their contact information is NOT shared with military recruiters.
